I understand that physical exercise carries inherent risks and agree that the participant will follow coaching instructions and take part within their ability.

If the participant is under 18, I confirm that I am their parent/legal guardian and give permission for them to participate. I consent to appropriate first aid being given and, where necessary, emergency services being contacted.
